Dear Heritage Families of Student Drivers:

22-23 Parking Permit Agreement In addition to the completed and signed form, you must also provide hard copies of the current information listed below and attach to this form for our files. If you are not able to make copies at home, you may make copies at the library.
 
 
We will need the following before issuing a parking pass for the 22-23 school year:  
CURRENT parking form
CURRENT driver's license
CURRENT proof of insurance 
CURRENT registration
 
Payments are available through the bookkeeper, cash or check only. 
In addition to the parking permit fee of $50.00 for the whole year or $25.00 for the second semester, all past fees must be paid and current.  Payments are available in person with the bookkeeper, cash or check only. All costs of permits are final and will not be refunded.
