Phoecix Program(10th-12th) is a program that serves expelled students and students at risk of expulsion. If you have questions about this program or you are interested in learning more about it, please contact an administrator or counselor at your LPS home school. Prior to enrollment, all students participate in an intake process that includes the student and their parent/guardian(s).
These forms should be filled out by the students home high school only.

To assist in meeting the needs of the student being referred to Phoecix Program(10th-12th), a minimum of TWO instructional teachers or staff members that have worked closely with the student MUST fill out the above form.
